自閉症孩童之相互注意協調能力治療

相互注意協調能力(Joint Attention)

壹、相互注意協調能力之典型及非典型發展

  自閉症患者的相互注意協調能力(Joint Attention, 簡稱JA)發展存在特異的障礙,相較於其他障礙,自閉症孩童更少有主動引發相互注意協調的行為,如注視、指出、展示,且較少對成人做出反應。因為相互注意協調能力的影響層面極大,且和自閉症患者是否可成功適應社會相關,是為臨床介入自閉症孩童的重點。

  為闡明自閉症孩童相互注意協調能力缺損對發展的影響,以下簡介相互注意協調能力之典型發展與非典型發展。

一、相互注意協調能力之典型發展

  促進相互注意協調能力的發展需要以下三項能力:(1)共享注意力(Sharing Attention)、(2)共享情感(Sharing Affect)、(3)共享意圖(Sharing Intentions)。

(一)共享注意力(Sharing Attention)
  在典型發展中,共享注意力一般從出生發展至一歲。在起初數個月,照顧者會時刻注意著嬰兒的視線,此時嬰兒和照顧者會藉由雙向互動分享注意力。9個月時,孩童會積極地觀察他人,學會在人與物品之間轉移視線,以確認照顧者是否注意到孩童感興趣的物品。這個過程也被稱為協調共同參與(Coordinated Joint Engagement)或社會參照(Social Referencing)。視線轉移在調節社交互動中扮演著相當重要的角色,因為這意味著對同伴展現注意訊號和社交興趣。在一歲前,孩童可在照顧者看向或指向感興趣的東西時跟隨照顧者的焦點,這一過程被稱為跟隨或回應相互注意協調能力的注視/指出;這是察覺他人的視覺觀點和意圖的基本能力。

共享注意力(Sharing Attention)

(二)共享情感(Sharing Affect)
  共享情感指的是對他人表達情感狀態,如:孩童表情愉悅並注視照顧者以分享正向經驗,或向照顧者表現出不適或困擾的信號以尋求安慰。透過與照顧者共享情感,孩童也從照顧者的回應中學會理解他人的情感狀態。

(三)共享意圖(Sharing Intentions)
  共享意圖,是指能夠向他人發出信號或直接的行為,以達到特定的目標。約9-10個月時,孩童開始使用聲音、手勢、和其他溝通意圖的行為。在早期階段的發展,共享意圖包含協調共享注意和/或使用手勢和聲音向他人表達意圖的影響。
共享意圖(Sharing Intentions)
  在一歲前,孩童主要會表達三種意圖,稍後會透過字句出現:

1.行為調節(Behavioral Regulation)
  使用信號以調節他人的行為,像是請求物品、行動、抗拒物品或他人行為(例如:指向要求的食物、推動物品以抗拒)。

2.社交互動(Social Interaction)
  使用信號以吸引別人的注意,例如:問候、呼叫、請求社會常規和要求舒適感(例如:揮手「再見」、獲得安慰)。

3.相互注意協調(Joint Attention)
  使用信號將他人注意導向至有趣的物品和事件以分享(例如:為展示感興趣的物品,指向該物以吸引他人的注意力)。

  當結合了共享注意、情感和意圖三者後,即可發展更廣泛的分享經驗能力。此時孩童開始明白,他人有自己獨特的思想和心智,且可利用溝通來分享個人意識。這樣的理解被稱為互為主體性(Intersubjectivity),或主觀經驗分享,出自於孩童自發地嘗試向照顧者分享經驗。互為主體性需要有共同意義的手勢、面部表情、聲調以及語言。分享經驗的能力以交互互動做為基礎,使得孩童的行為和目標更為細微。

二、自閉症孩童之相互注意協調能力非典型缺損情形

  在所有臨床症狀中,相互注意協調能力缺損是自閉症患者在發展中最早顯現的障礙,且通常在一歲以前出現,也是自閉症患者的特有病徵。

  自閉症患者在嬰幼兒時期即出現相互注意協調能力缺損,較少出現分享注意力(Attention Sharing)、手指指示、展示等行為,且較少對照顧者產生回應,並經常以直接的身體接觸(例如:拉實驗者的手)來尋求協助。

  相互注意協調能力對孩童的早期發展有廣泛的影響,且關係到自閉症患者兩大主要障礙:語言和社會發展。以下分別概述自閉症患者與相互注意協調能力缺損相關之障礙。

(一)語言溝通障礙
  相互注意協調能力是學習語言時必要的分享經驗基礎。自閉症孩童以手勢展現的相互注意協調能力(Gestural Joint Attention Skill)如:展示或手指指示行為,可顯著預測其語言之發展。進一步解釋是語言的學習(例如:詞彙)是在相互注意協調能力的片段中學來的。

(二)情緒障礙
  相互注意協調能力是一種兼具認知及情感的分享性溝通行為。自閉症患者在表達情緒上較困難且難以被理解,當要求自閉症患者做和情緒相關的臉部表情及聲音時,自閉症患者表現得相當怪異,以致於一般人難以分辨。大部份的自閉症幼兒對成人的負向情緒經常是沒注意到或是忽略,他們在成人出狀況時(例如:不小心弄傷了手、害怕怪異玩具等),並不會注意成人,反而繼續玩自己的玩具。

(三)社會互動障礙
  社會動機是構成相互注意協調能力的基礎,而相互注意協調能力則被視為對社會瞭解的開始。Mundy等人(1994)研究發現不論一般或自閉症孩童,出現頻率較高的相互注意行為與父母的正向行為(包括視線接觸、情感表達、和模仿)成正比。

(四)模仿及角色扮演障礙
  相互注意協調能力是瞭解他人溝通意圖及角色模仿的基礎,也和假裝遊戲有關。自閉症孩童無論是口語或手勢的模仿皆有困難且在角色模仿能力也有所缺陷,於自發性假裝遊戲也有明顯的障礙。

貳、相互注意協調能力之治療途徑

  雖然近代學者對於相互注意協調能力的定義歧異,但普遍可被定義為兩種類型,一為主動相互注意協調能力(Initiating Joint Attention, 簡稱IJA),二為回應相互注意協調能力(Responding to Joint Attention, 簡稱RJA)。IJA指為嬰兒使用眼神接觸、影響力、手勢、聲音或符號溝通以自發性地分享正向影響或興趣;RJA被定義為孩童遵循成人注意力指示。照顧者和孩童藉由輪流主動和回應共同建立社交對話,立足於共同關注的焦點以相互交流。

  目前臨床一般介入相互注意協調能力之治療途徑有:發展回應途徑(Developmental-Responsive Approaches)、單一嘗試訓練途徑(Discrete Trial Training Approaches)、認知社會交流途徑(Transactional Approaches)、綜合途徑(Combined Approaches)。

  當設立治療目標時,關鍵在於確認促進的是IJA、RJA或是兩者並行。在過去的研究中,認知社會交流途徑有希望促進IJA,而促進類化和維持IJA至少需要6個月每週20小時的治療;綜合途徑則有希望促進RJA,為了促進類化和維持RJA,須在與自閉症孩童眼神接觸時命名或示範使用物品的功能,若要長遠維持習得的RJA技能,則需要改變親子溝通模式。

一、認知社會交流途徑

  認知社會交流途徑(Transactional Approaches),主要目標在於IJA。認知社會交流途徑假設兒童可學習將社會連結視為回饋(例如:看到媽媽走過來很高興),並表現成人示範的IJA行為(例如:在媽媽到達時,微笑指著媽媽)。它們被設計在類似遊戲、互動的框架下使用漸進提示IJA的有效形式,可能因此增進社交互動的品質。對許多孩童而言,促進類化和維持IJA至少需要6個月每週20小時的認知社會交流途徑。

  認知社會交流途徑的兩個例子是社會溝通、情緒調解和交流支持模式(Social-communication, Emotional regulation, and Transactional Support Model,簡稱SCERTS),和反應性教育與語言前自然環境教學(Responsive Education and Prelinguistic Milieu Teaching, 簡稱RPMT)。兩者的主要社交同伴都須對孩童的溝通行為及時反應才有效。認知交流途徑跟隨孩童的注意和遊戲引導,參考發展階段以循序習得能力,並嘗試建立類似遊戲且輪流的常規。

  為了避免使用提示的潛在負面影響,可參考以下提示的原則:為了增加孩童維持與成人一同參與活動及對物品的興趣,只在孩童參與動機高昂,且有開啟溝通的行為後提示(例如:投入於輪流活動);為了避免依賴提示,須及時給予最少協助的提示。

  例如,當孩童給老師物品時,老師可以下列順序以提示孩童以注視協調:
1.當孩童未注視時,教師的反應延遲。
2.如果孩童還是不看成人,可說:「看我。」
3.攔截孩童的視線。

  這些重複的提示只在孩童溝通動機高昂時進行。例如,孩童投入輪流活動時(例如:玩滾球迷宮的連續第三個回合);一旦孩童達到一定的提示要求時,需逐漸減少提示;最後,少使用肢體引導,避免脅迫方式要求孩童作出回應,鼓勵以IJA表達正向情感(例如:微笑),可在適當的情境下示範宣告的行為(例如:慶賀把積木撞倒了的結果),對於孩童的失敗應避免使用負向回應。

二、綜合途徑

  綜合途徑(Combined Approaches)結合了認知社會交流途徑(Transactional Approaches)和單一嘗試訓練途徑(Discrete Trial Training Approaches)的元素,創造了一個新的治療方式。

  在進行行為順從度訓練(Behavioral Compliance Training)後,才進行RJA訓練,教導孩童回應注意力導向提示。在學習回應更多細微的提示前(例如:成人注視目標物),教導孩童要回應近處、大量的注意力導向提示(例如:成人將手放在目標物上)。成人需先提供取得玩具的機會以交換雙方的眼神接觸,再要求孩童回應更多近端注意力導向提示前(例如:展示目標物及指向目標物)。

  此途徑目標為孩童可以被成人指定的物品所吸引(視覺注意或觸摸)至少五秒鐘。正確回應的回饋為可玩玩具(例如:正增強)。若孩童反應不如預期,將撤走所有的玩具(例如:使用隔離做為負增強)。在兩次不正確的反應後,成人會以肢體提示孩童去觸摸(同時注視)新玩具。精熟的標準為五次連續療程中有四次80%順從注意力導向。

  在掌握RJA訓練後才進行IJA訓練,目標為可協調地轉移視線和指出物品。孩童玩玩具若超過10秒,則可能產生IJA。若IJA並未在前一個IJA發生後10秒鐘內出現,則玩具會被撤走。發生兩次後,須肢體引導孩童移動物品並注視成人的眼睛。成人同時會說:「給我看。」並指向他的眼睛做為額外的提示。隨著視線轉移增加,則提示下降。精熟的標準為五次連續療程中有四次至少有30%自發性地在物品及成人間轉移視線。30%的標準是基於典型發展的孩童的觀察。

  當使用綜合途徑,需要密切注意維持RJA的進步。可以透過教導成人漸進提供不同回饋,且循序漸進至較遠端的注意力提示。此途徑的訓練時間雖足以誘發RJA(約 30個小時的訓練),但對維持新的RJA技能時間並不明確。若要支持有效的新RJA技能維持,改變親子溝通模式可能是更長遠的訓練。

參、結語

  無論使用何種治療方法,高頻率和更長的治療時間是有效促進類化和維持相互注意協調能力的關鍵。家長和其它成人佔了鼓勵自閉症孩童IJA和RJA發展的重要角色。這兩種類型的相互注意協調能力須維持典型及功能性在發展上才有意義。為了類化及維持IJA,對於孩童的IJA嘗試行為家長一定要給予社交回饋,並對於其感興趣事物提供相關口語依據,以幫助孩童表達出注意到的事物。為了類化和維持RJA,家長需要在孩童轉移視線至成人的焦點時,命名或示範使用物品的功能。

  在治療時須清楚區分要訓練IJA還是RJA。且我們必須選擇符合自然環境下的相互注意協調能力的治療途徑或治療要素。最後,要衡量治療是否成功,需要持續測量類化和維持新的相互注意協調能力技能。
